and do the same thing over and over again. They can do that a long time, 20 hours or so, and the roundtrip takes 10 minutes. The result is that LDL is inside the cell, where it is degraded, the protein and the cholesterol is transported to its destination and so on. This is the normal picture, if anything goes wrong with the LDL receptor, there is no substitution for this process and that leads to very bad diseases called Familial Hypercholesterolemia, the name is bad enough. And this disease, this kind of genetic disease, or part of the patients that have this disease have mutations in the LDL receptor and there’s a database where you can find more than 1,000 different mutations. And if you think about the fact that there is only 839 residues in the human LDL receptor, that means that there is more mutations already found in patients than the whole protein has aminoacids. And the occurrence is quite high, 1 in 500 people are heterocygotes, that is they have a mutation of one of their chromosomes and they have usually early onset heart disease and arthrosclerosis and they have about sort of 400, 500 mg/dl cholesterol concentration in their blood. There is less than 1 in a million homocygotes, and these people are in very poor shape, they rarely get older than 20 years or so and develop heart disease at a very early age. So we set out to determine the structure and if you look at this scheme, which is sort of a schematic of the organisation of this protein, 839 residues, starting with seven so called ligand binding domains, each about 40 residues, then EGF-like domains called A, B and C and between B and C a beta propeller and then a region that has carbohydrates bound to it, then a membrane spanning region and a cytoplasmic small region, about 50 residues. If something goes wrong or something is mutated in this part, then usually the concentration of receptors in pit, in coated pits doesn’t work well and the import is inefficient. But many of the mutations are in this part. But, as I said, when you look at this, it takes a certain amount of craziness to try crystallise this protein because it consists of sort of autonomous domains that may resemble beads on a string and should be very, very flexible. However, it turned out that at low pH the protein could be crystallised and, I jump over many years of frustration and hard work, especially by Gabby Rudenko, here is the result showing a 3.7 Å resolution electron density map of the receptor. Because other people had determined structures of single domains, especially this part here, the beta propeller by Steve Blacklow and his colleagues. And also some of the ligand binding domains, we could nevertheless interpret this electron density using the known structures of the fragments. And it showed a picture that is very different from what I ever imagined the receptor to be. The first ligand binding domain is invisible and then there is this beads-on-a-string kind of structure with the 7, the other 6 ligand binding domains, then the A- and B-EGF and the C-EGF and the beta propeller. And there is a very, very tight contact between the ligand binding domains 4 and 5. There is also some carbohydrate visible from the protein that is heavily glycosylated. And here is another view, just the alpha carbon trace and the main results independent of each other, the ligand binding domains, they fold back on the beta propeller, the R4, R5 domains dock on the propeller and this lower part looks like a rigid domain. And if we look at the interface between the beta propeller and the R5 and R4, we can see that there are, first of all, a lot of conserved residues, marked by a C, then residues that have been found in hypercholesterolemia patients to be substituted, to be mutated. And two of them actually were deleted during the mutation. So this is a hot candidate and, especially in the centre of the picture, there are histidines that almost certainly change their charge state when you go from pH7 to pH below 6. And Steve Blacklow has done the calculation showing the surface at pH7 with the electrostatic potential projected on the surface. Red is negative, blue is positive and there is a big difference, a patch here where these histidines are that change the charge state. And that may be the switch that tells the receptor to let go the LDL and use itself as a ligand. And so the idea is that at neutral pH the receptor is really a loose structure, it is able to follow the surface of the LDL particle. When the pH drops, most likely the binding weakens between the receptor and the LDL and the receptor finds an alternative confirmation that doesn’t require LDL and the LDL is free to go. Anything that keeps the receptor from releasing LDL leads to a futile cycle of transport, essentially to an ineffective system of cholesterol homeostasis. There would be a lot more interesting things to look at in cholesterol homeostasis, especially the mechanism by which these membrane proteins, for example HMG-CoA reductase or a protein that controls the transcription of a large number of genes responsible for cholesterol production and cholesterol import, how the mechanism of cholesterol sensing is working and we are at some point on the thorny pathway of making crystals of these and there is no way to predict if or when we will be successful. And again, also we need every help from every excellent student we can get. Here is a list of the people involved and I think I mentioned most of them. Und wenn wir uns vor Augen halten, dass es lediglich 839 Reste im menschlichen LDL-Rezeptor gibt, bedeutet dies, dass bereits mehr Mutationen bei Patienten gefunden wurden, als das ganze Protein Aminosäuren besitzt. Und die Häufigkeit des Auftretens ist recht hoch, eine von 500 Personen ist heterozygot, weist diese Mutation also auf einem ihrer Chromosome auf, und diese Menschen leiden in der Regel bereits früh an Herzkrankheit und Arteriosklerose und ihre Cholesterinblutwerte liegen bei um die 400, 500 mg/dl. Es gibt weniger als eine homozygote Person in einer Million, und diesen Leuten geht es sehr schlecht, sie werden selten älter als 20 Jahre oder so und entwickeln schon in sehr jungen Jahren Herzkrankheiten. Versuchen wir also, die Struktur zu bestimmen, und wenn Sie sich diese Abbildung ansehen, die eine Art schematischer Darstellung der Organisation des Proteins darstellt, 839 Reste, angefangen bei sieben sogenannten ligandenbindenden Domänen, jede mit etwa 40 Resten, dann EGF-homologe Domänen, A, B und C genannt, und zwischen B und C ein ß-Propeller und dann eine Region, an die Kohlenhydrate gebunden sind, dann eine Transmembranregion und eine kleine, zytoplasmatische Region mit etwa 50 Resten. Wenn etwas schief geht oder etwas in diesem Teil mutiert, stimmt gewöhnlich die Konzentration von Rezeptoren an bestimmten Stellen, den Coated Pits nicht und der Import ist ineffizient. Viele der Mutationen sind jedoch in diesem Bereich zu finden. Aber, wie schon gesagt, man muss schon ein wenig verrückt sein, um zu versuchen, dieses Protein zu kristallisieren, denn es besteht aus einer Art autonomer Domänen, die Perlen auf einer Schnur ähneln und sehr, sehr flexibel sein sollten. Es stellte sich jedoch heraus, dass bei einem niedrigen pH-Wert das Protein kristallisiert werden konnte und hier ist das Ergebnis, es zeigt eine Elektronendichtekarte des Rezeptors mit einer Auflösung von 3,7 Angström. Da andere Leute die Strukturen einzelner Domänen bestimmt hatten, insbesondere dieses Bereichs hier, Steve Blacklow und seine Kollegen den ß-Propeller, und auch ein paar der ligandenbindenden Domänen, konnten wir mithilfe der bekannten Strukturen der Fragmente dennoch diese Elektronendichte interpretieren. Und es zeigte uns ein Bild, das sich sehr von den Vorstellungen unterschied, die ich mir von dem Rezeptor gemacht hatte. Die erste ligandenbindende Domäne ist nicht sichtbar und dann kommt diese perlenkettenartige Struktur der 7, der 6 anderen ligandenbindenden Domänen, dann die A- und B-EGF und die C-EGF und der ß-Propeller. Und es besteht ein sehr, sehr enger Kontakt zwischen den ligandenbindenden Domänen 4 und 5. Man kann auch ein paar Kohlenhydrate des stark glykosylierten Proteins erkennen. Und hier ist eine andere Ansicht, nur eine Backbone-Darstellung der Alpha-Kohlenstoffe und der Hauptergebnisse, unabhängig von einander, die ligandenbindenden Domänen, sie bilden eine Schleife zurück zum ß-Propeller, die R4-,R5-Domänen docken an den Propeller an und dieser untere Teil sieht wie eine starre Domäne aus. Und wenn wir uns die Schnittstelle zwischen ß-Propeller und R4 und R5 ansehen, erkennen wir zuerst eine Menge konservierter Reste, durch C gekennzeichnet, dann Reste, die bei Hypercholesterinämie-Patienten ersetzt, mutiert waren. Zwei davon waren durch die Mutation gänzlich verschwunden. Dies ist also ein heißer Kandidat, insbesondere in der Mitte der Abbildung gibt es Histidine, die beinahe sicher ihren Ladungszustand ändern, wenn Sie den pH-Wert von 7 auf unter 6 absenken. Und Steve Blacklow hat dies berechnet, dies zeigt die Oberfläche bei pH 7, wobei das elektrostatische Potenzial auf die Oberfläche projiziert ist. Rot ist negativ, blau ist positiv und es gibt einen großen Unterschied, eine Stelle hier, an der sich die Histidine befinden, ändert den Ladungszustand. Und das könnte der Schalter sein, der dem Rezeptor mitteilt, das LDL freizugeben und sich selbst als Liganden zu verwenden. Und daher geht man davon aus, dass ein Rezeptor bei neutralem pH-Wert tatsächlich eine lose Struktur bildet, dass er in der Lage ist, der Oberfläche des LDL-Partikels zu folgen. Fällt der pH-Wert, schwächt sich wahrscheinlich die Bindung zwischen Rezeptor und LDL ab und der Rezeptor findet eine alternative Konformation, für den kein LDL erforderlich ist, und das LDL kann sich lösen. Alles, was verhindert, dass der Rezeptor das LDL freigibt, führt zu einem nutzlosen Transportzyklus, im Wesentlichen zu einem ineffektiven System der Cholesterinhomöostase. Es gäbe weit interessantere Dinge, die ich Ihnen bei der Cholesterinhomöostase zeigen könnte, insbesondere den Mechanismus, mit dem diese Membranproteine, zum Beispiel die HMG-CoA-Reduktase oder ein Protein, das die Transkription einer großen Zahl von Genen steuert, die für die Cholesterinproduktion und den Cholesterinimport verantwortlich sind, wie der Mechanismus der Cholesterinerkennung funktioniert. Wir befinden uns auf einem dornenreichen Weg, Kristalle davon herzustellen, und es gibt keine Möglichkeit vorherzusehen, ob und wann wir Erfolg haben werden. Abstract

Cholesterol is both an essential component of cell membranes, and it is required for the synthesis of bile acids, steroid hormones, and other important compounds. The human body obtains the needed cholesterol either by intracellular synthesis or by uptake with the diet.

An obligatory step in the intracellular synthesis of cholesterol is the conversion of 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) into mevalonate and CoA by the enzyme HMG-CoA reductase; two molecules of NADPH deliver the four electrons required for this reaction. Human HMG-CoA reductase is a polypeptide chain of 888 amino acids; it includes a membrane-spanning portion (a.a. 1-350) and a catalytic portion (a.a. 460-888), connected by a long linker polypeptide. The crystal structure of the catalytic portion shows a tetramer that consists of two dimers, each of which forms two active sites. The active site structure of the human enzyme differs markedly from that of a bacterial HMG-CoA reductase.

Drugs such as the widely used statins can inhibit the synthesis of mevalonate and thus cause an increased uptake of cholesterol from the bloodstream. Crystallographic studies of the catalytic portion of human HMG-CoA reductase with bound statins show the drugs bound to the active sites of the enzyme. The statin molecules not only occupy the binding site of HMG-CoA, but also prevent the formation of the complete active site configuration necessary for the enzyme's function.

Cholesterol is transported in the bloodstream mostly in the form of low density lipoprotein (LDL), a complex of phospholipids, cholesterol, cholesteryl esters, and apolipoprotein B. The LDL receptor, a membrane anchored protein of 839 amino acids, mediates the uptake of LDL into the cellular interior. This receptor binds LDL at neutral pH with high affinity, and releases it when the pH drops below ~6, as happens in endosomes during endocytosis. A crystallographic study at 3.7 Å resolution of the extracellular portion of the receptor (amino acids 1-699) in its low-pH form shows that two of the seven LDL binding modules form a tight contact with the so-called beta-propeller region of the receptor molecule, thus preventing ligand binding. This suggests a possible mechanism for ligand release by the LDL receptor: at low pH the receptor develops a new binding site for the ligand-binding modules which then competes with the ligand LDL. Mutations that are likely to disturb this mechanism have been found in patients with the genetic disease hypercholesterolemia.
